Home Care Coordinator
Responsible for the administration and coordination of all Home Care services provided to seniors as part of the LIFE program. The Coordinator must work effectively with LIFE staff as well as outside staffing agencies to meet the needs of participants and their families and ensure consistent quality care delivery in the home and the LIFE center. Excellent clinical skills and the ability to be flexible and respond to change quickly are a must for the position.
Executive Assistant
Assist the Executive Director in the efficient operations of the LIFE program. Supervises administrative and clerical staff. Assure that proper administrative procedures are maintained at all times. Carries through to completion specific work projects assigned by the Director, relative to the operation of the LIFE program and sites and to provide information for evaluation and revision of regulations, procedures and practices to assure that proper procedures are maintained. Performs LIFE program Human Resources' duties including record keeping administrative detail and follow-up.
Responsibilities:
- Organizes
and maintains the administrative office:
- Performs administrative duties for the executive director.
- Greets visitors to the administrative offices; maintains schedule of meetings, organizes and distributes agendas, attends meetings, prepares and distributes minutes.
- Prepares purchase requests, orders and invoices; gathers information and prepares confidential and special reports; assists with billing; assures completion of letters, reports and memos in proper format.
- Supervises and manages administrative assistants, receptionist and other clerical staff.
- Acts as campus liaison to the Central Office HR Department for HR functions including: organizing and maintaining medical and personnel files, being responsible for new hire orientation and required paperwork, and completing personnel forms and inquires received from internal and outside sources.
Requirements:
- Associate degree, business school graduate or its equivalent.
- Three years administrative office experience.
- Human Resources experience preferred.
- Intermediate level computer skills with knowledge of Microsoft Office Suite.
- Effective communication in person, in writing and on telephone.
- Able to qualify for and meet Notary requirements.
- Capability to lift and support up to 25 pounds and push/pull up to 200 pounds.
- Valid PA driver's license and a good driving record.
Social Worker
Part-time. Responsible for promoting and maintaining the health of individuals in the LIFE community through coordination, counseling, intervention, and appropriate preventive services. Responsible for assessment of participant needs and contributing to the plan for care of the individuals and families. Provides casework services to participants and assists with the admission process. Must possess a Bachelor's or Master's degree in Social Work from an accredited school of social work; social work experience in a hospital, long term care facility, or community setting; and one year of experience working with a frail or elderly population.
Home Care Aides
Full- and part-time. Provides physical assistance and attendant care to participants across care settings (Center, home, transportation). Provides assistance with nursing care, restorative therapies, personal care, activities, and meals across care settings. Demonstrates the knowledge and skills necessary to provide services to frail elderly participants according to assigned responsibilities and Albright LIFE standards.
CNA/Program Assistant
Provides physical assistance and attendant care to participants across care settings (center, home, transportation). Provides assistance with nursing care, restorative therapies, personal care, activities, and meals across care settings. Acts as a back-up driver; delivers competent and safe transportation services to all LIFE participants.
Driver
Full- or part-time. Provides safe and timely transportation services to Albright LIFE participants to and from home, Adult Day Centers, appointments, and other destinations deemed appropriate by the multidisciplinary team. Delivers and installs Durable Medical Equipment (DME).
